Individual planningFollowing a holistic assessment, the Occupational Therapist (OT) and the individual and/or others involved in the individuals care, will discuss the identified areas of ability, difficulty and need. The OT will collaborate with them in order to set realistic goals and to build a tailor-made treatment package. Intervention following the holistic assessment might include a graded rehabilitation programme to facilitate skills, function and confidence or compensatory strategies to facilitate independence and quality of life. The therapist will also work alongside any others involved with the individual's treatment to ensure a seamless, integrated approach.
